Do you see kids or just adults?

We see ALL ages! Dr. Scott is a board certified family practitioner. We see two day old babies to 100+


You don't take my insurance, can i still see you?

Yes! The cost for the visit is $60 (This includes everything)


I am not able to call during business hours, can I text or email you?

Of course! We understand many of our patients work from home, have kids and other obligations during the day. Please feel free to email us on our secure HIPAA email.


What is your cancellation/ no show policy?

Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. There is a $50 fee for all missed appointments. No shows and appointments, absent a compelling reason are not cancelled with 24 hour advanced notice.
No show fees will be billed to the patient. This fee is not covered by insurance and must be paid prior to your next appointment.
Consecutive no shows will result in a deposit being required upon rebooking, which you will get back at your appointment.
Multiple no shows in any 12 month period many result in termination from our practice.

FMLA/Disability Forms

There will be a fee for disability forms to be filled out ranging between $25-$50 depending on the length and detail of the paperwork. The patient is responsible for informing the staff at the time of scheduling that there will be forms to fill out. If we are not aware there is paperwork before your appointment, you may be rescheduled on another day, as we need to give you the proper amount of time to discuss the forms.
